At first glance, Sailor Chaos bears a resemblance to Sailor Galaxia in appearance only because of the similar headdress. However, upon closer inspection it is not the same headress as Galaxia (who isn’t the only one to wear a headdress or have long/semi-long hair.) Additionally, in the manga Sailor Galaxia and Sailor Chaos are completely different beings and not incarnations of each other. It is also known that Galaxia is NOT reborn as Sailor Chaos but that the being Chaos itself is reborn as Sailor Chaos. Sailor Chaos has what appear to be live snakes attatched to her fuku and appears to have at least one moving arm resembling a tentacle.
Sailor Chaos has two previous known incarnations, one in the past (metallia, the first known incarnation) and one in the future (Wiseman). After Metallia’s death at the hands of Sailor Moon in the first arc, it is revealed in the second arc that she was reborn as Wiseman in 30th century Crystal Tokyo (who was also later killed by Sailor Moon after his skeleton merged with the planet Nemesis).
Pharoah 90 and Nehelannia may have been influenced by the evil of Chaos in the manga but were not directly proved to be physically linked with Chaos through birth, as Nehelannia was from a far distant place outside the Solar System and Pharoah 90 was from another dimension entirely.
First Anime
A being referred to as “Chaos Galaxia” appears in the 90’s anime adaptation of Sailor Moon. She appears almost identical to Sailor Galaxia, albeit with an altered color palette and leathery bat-like wings.
